QUESTIONNAIRES: This request combines *many* of the same requests saying the same thing

Many feature requests say this… “Create a questionnaire from scratch” should NOT populate with 10 questions. It should truly be from scratch. It’s annoying to hide and scroll past all of the autopopulated questions. This really should be a 5min code change to remove those questions and just have a blank survey creator.
